even when domain is required if you put that requires domain it still doesn't ask for the domain name.

Hi, sorry for the delay i didnt see this new post.
The option has to be set on the order form also so that is shows up on the order form.

We are currently working on version 2 of the software, which is a complete rewrite. The new version will resolve all the issues like this in version 1. Version 2 will also be mostly open source, only a few files will be encrypted.
